UC Immunization History and Tuberculosis Screening Documentation Requirement

Per federal and state regulations we are prohibited from discussing or receiving private health information via non-secure emails. Please direct all inquiries to the Immunization Provider via "Messages" on the Health e-Messenger secure messaging system. See the Student Health Center website for specific details.

Submission of the Immunization History and TB Risk Assessment on-line forms alone does not guarantee compliance with UC Immunization Requirements. These requirements are very specific (i.e. number and spacing of vaccines in a series) to attain compliance. Please review the specifics at:

After submission of your data, to check that your information meets the specific requirements, go to Immunization>Compliance Details within the Health e-Messenger site. Be sure to check the replies in Health e-Messenger from the Immunization Provider if you submitted any questions.


Parental Questions

Q: If parents have sent a question to the Student Health Center or the Insurance Office, how will they get a response?
A: General questions from parents can be answered with a telephone call; however, questions specific to their student must go to or through the student. The student may give the Student Health Center written permission by completing and submitting the Authorization for Release of Information form for Student Health Center staff to speak to the parent. If your student agrees, please have the student submit this authorization form.


Q: If my student does require any medical services while they are there, do you bill Blue Cross directly? Are you considered a provider? 
A: 
 Students with UC SHIP will have any claims sent to Anthem Blue Cross and/or Anthem Blue View Vision. UCSC Student Health Services only bills and submits to UC SHIP. Any other insurance, including other Anthem Blue Cross, plans are NOT submitted for billing. Students will get statements that are suitable for submitting to insurance carriers; however, UCSC Student Health Services is not a member in any other network.


Q: If we do not purchase campus insurance, where does my student need to go for medical care?
A: Please check with your insurance carrier for coverage and providers in Santa Cruz County.
