Formal Request for Social Security Numbers from Wells Fargo Insurance Services -
To comply with the Affordable Care Act, the UC SHIP insurance plan, Anthem Blue Cross, via our insurance broker, Wells Fargo Insurance Services, will be requesting Social Security Numbers so that they can submit required information proving that members have health insurance. Accept or Waive the UC Student Health Insurance Plan (UC SHIP) (all students).
All full and part-time undergraduate and graduate students are automatically enrolled in and charged for the UCSHIP plan unless they choose to submit an on-line insurance waiver confirming they have acceptable health insurance coverage.
IMPORTANT! UC Santa Cruz requires that all students have a primary care provider within 30 miles of their school address (on or off campus). Many students with Kaiser, Medi-Cal, or HMO insurance cannot meet this requirement. If you cannot identify a provider within this 30 mile radius, you may not qualify to waive the UCSHIP. All "approved" online waivers will be audited to ensure that UCSC's requirements are met.
If you have acceptable health insurance coverage and are sure that you want to waive the UCSHIP plan, follow these instructions:
- Review and complete the On-line Waiver Questions worksheet before beginning the waiver process.
- Navigate to the UCSC Waiver Site and complete the on-line waiver.
Waiver deadlines for 2016-2017:
Fall: 9/1/2016 Winter: 12/1/2016 Spring: 3/1/2017
2. IMMUNIZATION REQUIREMENTS
Starting fall 2016 all incoming students will be required to receive vaccinations and Tuberculosis(TB) screening and testing per the UC immunization requirements.
- Vaccinations and TB Screening and Testing must be completed prior to arrival on campus

Complete the online Personal Health History Form (all new students).
- Go to the UCSC Health e-Messenger web site.
- Log in and select “Forms”, then choose “Personal Health History” to complete and submit these secure online forms.
Obtain your medical records if you are being followed for a medical condition.
If you or your doctor want the UCSC Student Health Center to provide care or monitor your medications or lab work, please ask your primary care provider to compose a summary of your medical treatment plan. Please DO NOT send your entire medical record.
Records can be faxed to our Medical Records department at 831-459-3546 or mailed to UC Santa Cruz / Student Health Services at return address above.
