Dear Student,
Your 1095-B form for the tax year 2022 is ready to view electronically through your secure online Academic HealthPlans (AHP) student account.
Students enrolled in the University of California Student Health Insurance Plan (UC SHIP) will no longer automatically receive a paper copy of the 1095-B form due to a change in the tax law. As of 2020, providers are no longer to furnish physical copies of the 1095-B form to those issued insurance coverage, because there is no longer a penalty for failing to show proof of health coverage.
While the IRS will no longer penalize individuals for failing to report health coverage, your state may still require proof of coverage. If you have any questions or concerns regarding your state’s law, please contact a tax professional.
The 1095-B is a tax form that reports the type of health insurance coverage you have, any dependents covered by your insurance policy, and the period of coverage for the filing year.
You can download your 1095-B Form by following the steps below:
• Sign in to your account at myahpcare.com
• If not previously elected, provide consent in your account profile to receive your 1095-B form electronically
• Click Download 1095-B Form
If you would like to request a printed copy of the form, click here to submit an online request.
A 1095-B form will be mailed to the address on record within 30 days of the date the request is received. If the address on record is incorrect, please provide the address you want it mailed to in your request for the printed copy.
