This email confirms you’re enrolled in UC SHIP for the upcoming school term. A health plan built for students, by students, UC SHIP provides you with the medical, pharmacy, vision and dental benefits you need to stay healthy and in school.
Remember, you have high-quality, affordable and convenient coverage that protects you from unexpected health care costs, whenever and wherever you need care – whether attending classes on campus or distance learning.
You’ll find all the information you need to get the most from your benefits on the UC SHIP website. Don’t miss these top 6 takeaways:

- Discover what you need to know about your medical, dental, vision and pharmacy benefits.
- A referral may be required for medical care. Please confirm before your appointment by visiting your campus health center website or contacting them directly.
- Your UC SHIP ID card is electronic, through the StudentHealth app. Visit the App Store or Google Play to download the app to your mobile device. That way, your UC SHIP ID card will be handy when you need care.
- Register with LiveHealth Online today, so you’re set up to see a doctor via video chat whenever, wherever. You won’t need a referral to get 24/7 care and prescriptions for things like rashes, stress and anxiety, minor cuts and scrapes, fevers, pink eye, sinus infections, and more, from board-certified doctors, counselors and psychiatrists.

About that form you’ll be getting in your (e)mail…
It’s called the 1095-B, and it shows you’re enrolled in a health plan that meets federal and state of California minimum health insurance requirements. It’s the law. And every year, we’re required to send the Internal Revenue Service the Social Security number (SSN) or name and date of birth for all students—and dependents—who are enrolled in UC SHIP.
If your SSN is not on file at your campus registrar’s office, you will receive an email from the UC SHIP enrollment vendor, Academic HealthPlans (AHP), asking you to provide it. Please follow AHP’s instructions for providing your SSN.
- Please visit myahpcare.com/register.
- Complete the registration form and click Register Now.
- Once you are logged in, click on My Account, then View/Edit My Info.
- Click Edit next to Student Profile.
- Update SSN field. If you do not have a SSN, please select the check box next to “Do not have SSN”. Follow the same steps for your dependents.
- Select the check box next to “1095 Electronic Consent” if you would like to receive you and/or your dependent’s 1095-B form(s) electronically.
- Click Save Changes.
If you decide to not receive your 1095-B electronically, a copy will be mailed to the last address on file with your campus registrar’s office.
